Health Support Worker

County Durham

Full Time (37.5 hpw), Fixed term 12 months

Salary of £18,200 to £18,450 pa, plus brilliant benefits!

At Home Group its critical we continue to deliver services for our customers during the COVID-19 outbreak, and our awesome colleagues are at the very heart of making that happen. We need Brilliant People like you to join us (and not just during this challenging time). We are doing things a little differently at the moment, so to keep you, our customers and ourselves safe well engage with you digitally where possible. 

 

We have a fabulous opportunity for you to join our awesome (and brand spanking new) team €œThe PHB Champion Service€. We will support our brilliant patients to understand more about Personal Health Budgets and help them to apply if they meet the criteria. The aim of PHBs are to reduce the interaction and use or dependency on local health services whilst empowering patients. 

 

Where our patient is in receipt of a PHB, well support them to make choices that will help to improve their physical and / or mental health.

We will work with them to create their own bespoke health support plan.

Our Patients mainly have mental health support needs and are under a section 117 aftercare. This means that they could be suitable for a PHB and our brilliant support! 

 

Our patients are based throughout County Durham and living in a variety of settings eg in their own personal homes, a hospital or community support. Well support them wherever they are based, and that means travelling to meet with them. One day you could be in our Newton Aycliffe office and then next in Lanchester Road Hospital.

 

The PHB Service is a brand new to Home Group and an exciting opportunity for us all to grow and develop in the health field. At the moment, the contract is only for a year but we really intend to prove its worth in that time, in the hope of getting further on-going funding. Thats why youll be such an important part of the team that we are building. Your energy and success will have a direct impact in making this a more permanent opportunity long-term.

 

About our team

We are building our fabulous team from scratch. You (like us) are super passionate about supporting people to reach their goals and celebrating their achievements. 

 

The PHB Champion Service will be part of a wider team managed by Louise. Louise has worked at Home Group 8 years now! Outside of work Louise loves a bit of shopping, especially if it involved buying new d©cor or home accessories! You will be joined by your new PHB colleagues and your sister team who look after customers with physical and mental health needs. To us we arent just colleagues were a work family. There will be 10 of us between the 2 teams we dont just love to support our patients, were there for each other too!

 

Working hours

In order to meet the needs of our patients, we need to be a bit flexible. This means youll work 5 days per week from Monday to Friday but your hours (which will be in the day time) may flex a bit. As were asking you to be flexible for us, we aim to give you the time you need to spend time doing the things that matter to you!

 

About you

Youre passionate and motivated to help vulnerable people make positive and lasting changes to their lives. Youll impress us with your ability to work on your own initiative, positively influencing people from all different backgrounds. You will have some experience in safeguarding, risk assessments, confidentiality, support plans, alongside working collaboratively.

 

We help change our patients lives for the better. Youll play your part by assisting in the delivery of tasks that are based on our patients personal support plans, and their needs and aspirations. Youll work with our Support Coordinator and external agencies to assess and engage support for our patients. Youll empower our patients to develop the independent living skills they need, to successfully move on. Together well make a difference!

 

To make sure that we keep our records straight, and customers safe, it's really important that you are technologically savvy (or at the very least not a technophobe!)

 

Experience

To make sure our customers are well supported, its important that you have worked in a health and social care environment before. Dont worry, we wont drop you in the deep end, well help to you understand how things work at Home Group!

 

As this is a community-based role, you need to be able to drive, as well as having access to a car insured for business purposes. Dont worry, well pay you expenses to cover this use!

 

When you join, youll need to have a valid Enhanced Check under the Disclosure and Barring Service. The great news is that we will pay for that to be done if you dont have a transferrable one.
