Brilliant outcomes through integrated health, housing and care. Thats when it hits home.

At Home Group its critical we continue to deliver services for our customers during the COVID-19 outbreak, and our brilliant colleagues are at the very heart of making that happen. We're doing things a little differently at the moment, so to keep you and our customers safe, well engage with you digitally.

Be part of something brilliant at Home Group, where clinical interventions work hand in hand with daily care and support.   Were delivering brilliant long term outcomes for people who have complex support needs through integrating health, housing and care.

We have a brilliant opportunity for you to join our Transforming Care team and develop our brand new trauma informed womens service in Leeds, supporting our customers who have complex support needs and a history of trauma including Learning Disabilities, Autism, a diagnosis of mental health conditions, personality disorders, Psychosis and PTSD.  They may also have a forensic background and support needs related to their behaviour, and may have experienced difficult and traumatic childhoods, chaotic adult relationships or may have children who they are estranged from.

As a compassionate and knowledgeable leader, youll develop the service to deliver high quality, cost effective and person centred support in line with CQC Fundamental Standards and regulatory requirements. This is an exciting opportunity to really shape this brand new service to be psychologically and trauma informed, underpinned by a framework of Cognitive Analytic Therapy.  Youll be enabling our customers to live safe, happy and fulfilling lives within the local community, who otherwise could have remained in locked mental health wards or privately run facilities.  How amazing is that?

Youll work with our wider multi-disciplinary team, including clinical colleagues and Positive Behaviour Support Workers, ensuring that high quality person-centred care is delivered in a safe, effective, caring and well-led way.  This will be in line with positive behavioural support and our Living Independently Feeling Enabled (LIFE) support practice model (well tell you more about this when we meet you!).  Youll ensure the highest levels of customer safety, supporting customers to live as independently as possible and integrate back into a community environment.

As an experienced Mental Health Nurse, youll lead all activity within the service as well as deliver training and regular coaching to enhance the knowledge and skills of our teams, unleashing their potential so our customers always receive the highest standards of care!

Youll report to Karen, our Head of Transforming Care who has recently joined us bringing a wealth of experience from her clinical and operational roles working across the NHS, Social Care and private sector.  She is passionate about Transforming Care, delivering a robust and balanced offer to our customers, families and commissioners.  Shes also a dual registered RNLD & RMN nurse and qualified in Applied Behavioural Analysis.  Outside of work youll find her keeping fit in dance aerobics classes, singing in a choir (which shes just recently joined) and discussing the latest book as a member of a book club (we wonder what theyre reading at the moment?).

Where and when youll work

Will be providing 24 hr support to our customers which will be on a rolling rota, so you and your team will provide support covering evenings, weekends and bank holidays.  Youll also be required to be on call. So we need you to be flexible, but in return well give you the time back that you need!

A bit about you

It goes without saying that youll be an experienced Mental Health Nurse with substantial experience of working within a mental health setting supporting customers in a psychologically and trauma informed way.  Youll have an excellent understanding of the Transforming Care agenda, relevant regulatory policies and procedures, including fundamental standards and CQC regulations.  Youll be brilliant at supporting and unleashing the potential of operational teams to work with customers through a person centred approach, actively supporting skills development and team reflective practice.

It would be fantastic if you have previous experience of working with customers who have a forensic background, personality disorders as well as some experience in the practical application of Positive Behavioural Support.  If not, dont worry as our experienced team are on hand to support you!  Youll ideally be able to drive and have access to a car for work as you'll be required to travel and work within your region.
