Supported Housing Health Coach

Bath, Avon

Permanent, Part Time (16 hpw over a minimum of 3 days)

Salary circa £19,000 (pro rata), plus brilliant benefits

At Home Group its critical we continue to deliver services for our customers during the COVID-19 outbreak, and our awesome colleagues are at the very heart of making that happen. We need Brilliant People like you to join us (and not just during this challenging time). We are doing things a little differently at the moment, so to keep you, our customers and ourselves safe well engage with you digitally where possible. 

 

We have a fabulous opportunity for you to join our awesome team. We support our brilliant customers to maintain a supported tenancy with the aim that they will be empowered to move on into their own independent accommodation.  This role focuses on supporting with their health and wellbeing goals to ensure a holistic approach. Our aim is to deliver long-term positive solutions for the people we work with and measure our success not by the number of people we support, but through the outcomes our customers achieve.   

We use an asset-based approach to help our customers to build their own personal resilience, improve lifestyle choices and prepare for independence by building on their own individual skills.  We work alongside our customers around their housing and health goals, and you will be delivering, amongst other things, our accredited, bespoke Home Achievement Programme.  This is a modular programme and customers complete modules in relation to housing and health gaining 9 credits which is comparable to a GCSE.  This is recognised locally by housing providers and supports our customers to move on to their own independent accommodation.

About our team

Youll be working alongside our fabulous team, who are super passionate about supporting customers to reach their goals and celebrating their achievements. To us were not just a team were a €œwork family€. Our manager is Karen, who has worked in this sector for 15 years, working her way from support worker to managing the services so she knows a thing or two about supported housing!

When not supporting her team to support our customers with their aspirations, Karen can be found stewarding for AMCA motocross (motorbikes to you or I!).

 

There are 4 of us in the team, and we are searching for you to be the 5th.  If you have a solid work ethic, common sense and a strong sense of teamwork, then that person may well be you!  If you have a good sense of humour and love cake, well thats a bonus!

 

Working hours

The office is open Monday to Friday between 9am to 5pm, so we can be flexible with when you work the 16 hours. As long as it is spread over a minimum of 3 days, Karen is happy to have a chat about your hours so it works for you as well as us.  

 

About you

Youre passionate and motivated to help vulnerable people make positive and lasting changes to their lives. Youll impress us with your ability to work on your own initiative, positively influencing people from all different backgrounds.  Customers needs vary so an understanding of homelessness, mental health and substance misuse is important.

 

As this is an evolving service, you will have the freedom to make the role your own and have your own print on developing it with and for our Customers. So working with our customers, you can develop and facilitate groups to develop their skills, from cooking, or healthy eating on a budget to arts and crafts to mindfulness. We have an in-house accredited course to offer our customers, which you will be involved with running.  Youll be imaginative and energised while remaining professional, strategic and outcomes focused, building on your confidence and skills to deliver a brilliant service to brilliant people.  Its a great opportunity to work in a very diverse and exciting service!

 

To make sure that we keep our records straight, and customers safe, it's really important that you are technologically savvy.  Youll be given the latest IT kit, which could include a Surface Pro tablet and smart phone, supported by our new online integrated IT system which means that you dont have to be in the office as you can complete support plans on the go.  We use the PAM (Patient Activation Measure) to measure distance travelled and a variety of person centred planning tools to ensure we really do put our customers at the heart of their support.  It would be brilliant if you have used computer based record keeping before but dont worry we will train you to use our systems and tools as part of your comprehensive induction.

 

It would be amazing if you have worked in this type of environment before, however its your €œcan do€ positive attitude and enthusiasm that are critical to make this role a success. We can nurture your talent, just let us know in your cover letter why you have a passion for working here! Finally, as the service is over two properties, you could be travelling to spend time with your customers. The properties are only a mile or two apart so you could do this role using public transport, walking, or if you drive and have access to a car that would also be amazing! (we pay your expenses to cover this use but we dont have parking at our main office!) 

 

If you are successful, we need you to have an Enhanced Disclosure Check through the Disclosure and Barring Service which well pay for.
