Mental Health Support Coordinator

Permanent, Full Time (37.5 hpw)

At Home Group its critical we continue to deliver services for our customers during the COVID-19 outbreak, and our awesome colleagues are at the very heart of making that happen. We need Brilliant People like you to join us (and not just during this challenging time). We are doing things a little differently at the moment, so to keep you, our customers and ourselves safe well engage with you digitally where possible. 
 
We have a fabulous opportunity for you to join our awesome brand new team. We support our brilliant customers who have mental health issues. Its very difficult to describe our customers as we have not met them yet! This will be a brand-new mental health service providing support to 26 individuals aged 18 and up. Our customers will be living in their own flat within our Buller Court complex. 
 
If you are up for the additional challenge of a new job role during Covid19 then you will be working closely with our fabulous and enthusiastic support workers and manage a caseload of customers. As a Support Coordinator you will be working in a person-centred way to manage your caseload and have the ability to empower our customers to achieve the goals that they have set. You will be a guiding star for our support workers and work closely with them to ensure that support delivered is of high quality. You will also play a key role in working with all external agencies; stakeholders; family members and really anyone else that our customers want involved in their support.
 
About our team
This is a really exciting time for us as the service is brand new so you will be joining alongside other newbies and go through your Induction together. You will be working alongside 4 Support Coordinators, 4 Support Workers, 1 Client Service Manager, 1 Mental Health Clinical Lead and of course our manager, Jay. The team will all be super passionate about supporting customers to reach their goals and celebrate their achievements.
 
To us we are not just a team were a work family. Our manager is Jay Gathergood, and she is in her eighth year working with Home Group. She is energised by the thoughts of future success of this service; and is in her first Senior Client Service Manager role! She has joined the service following three years as a Client Service Manager in Basingstoke and has 9 years of experience within this type of setting. When not at work you will usually find Jay listening to heavy metal music and borrowing other peoples dogs for walks! Alongside Jay there is also Support Coordinator Andrew Armstrong, our resident astronomer, and Support Worker, Steve Dombrick, animal lover and fellow metal music fan!
 
Speaking of Jay; she will be swanning off on her holiday's week commencing 22nd June (lucky devil!). She will be in touch with candidates on her return week commencing 29th June. For those of who are invited to interview the interview date will be 6th July.
 
Working hours
We provide 24-hour support to our customers each day and every day. Youll work on a rota basis and take your turn working days, evenings, weekends and bank holidays. We need you to be flexible to meet the needs of our customers, but we aim to give you the time you need to spend time doing the things that matter to you!

About you
Youre passionate and motivated to help vulnerable people make positive and lasting changes to their lives. Youll impress us with your ability to work on your own initiative, positively influencing people from all different backgrounds. 
 
Youll help change our customers lives (and our services) for the better by undertaking risk assessments, support planning, goal setting and regular reviews. Youll create bespoke and personalised support plans with and for your customers.  Each plan will look to meet our customers individual needs and aspirations, making sure they meet safeguarding and confidentiality requirements. Youll also have responsibility for coordinating our support workers, apprentices and volunteers, and working collaboratively with external agencies, to assess and engage support for our customers.
 
We empower our customers to develop the independent living skills they need, to successfully move on. Together well make a difference!
 
To make sure that we keep our records straight, and customers safe, it's really important that you are technologically savvy (or at the very least not a technophobe!) Youll be given the latest IT kit, which could include a Surface Go tablet and smart phone, supported by our new online integrated IT system which means that you dont have to be in the office as you can complete support plans on the go. We have found that use of our IT equipment is great for helping us adapt to the ever-changing Covid19 Government guidance too!
 
To make sure our customers are well supported, its important that you have worked in a supported housing environment before. Dont worry, we wont drop you in the deep end, well help to you understand how things work at Home Group! Your €œcan do€ positive attitude and enthusiasm will be critical to make this role a success.
 
You will not need a car to complete this role; but you will need to travel around to spend time with your customers. You could do this role using public transport, or if you drive and have access to a car that would also be amazing! (we pay your expenses to cover this use!).
 
When you join, youll need to have a valid Enhanced Check under the Disclosure and Barring Service. The great news is that we will pay for that to be done if you dont have a transferrable one.
