Birmingham, West Midlands
Permanent, Full Time (37.5 hpw)
Salary £19,000 to £20,125 pa rising to £23,500 through length of service

We have a fantastic opportunity for a brilliant Supported Living Worker like you to join our fab and supportive team. Our Good Rated CQC registered service provides support to our brilliant customers who have a mental health diagnosis.  Aviary House provides accommodation for individuals with a mental health diagnosis aged 18+ years old who require care within an accommodation support setting.  The property consists of 28 self-contained flats, four of which are respite flats for customers stepping up from the community or stepping down from hospital who are in crisis.
 
Youll help our customers identify their aspirations, and work with them to create plans that help them achieve these. You also help your colleagues to deliver on the plans youve created to achieve excellence. Our aim is to support our customers to live their day-to-day lives as independently as possible.
 
The team will tell you that no two days are ever the same because everything we do is €œperson centred€ around our customers. You might help with personal care (like bathing and issuing medication) in the morning and supporting them to attend medical appointments, shopping and trips out in the afternoon. As we work in a person-centred way, with our customers at the heart of every decision we make, your day will be based around the individual support plans making each day as unique as our customers.  Like our team, we are sure youll find the role ever so rewarding.
 
About our team

Were also super passionate about supporting customers and celebrating their achievements. To us were not just a team were a work family. Our registered manager is Deborah Duggan who is caring and compassionate with a wealth of clinical experience and great sense of humour.  There are 21 of us in the team and we dont just love to support our customers, were there for each other too! The team consists of Supported Living Workers, an Activity Worker, Supported Living Coordinators, a Senior Client Service Manager and the Registered Manager.
 
Working hours
Youll work on a rota basis between the hours of 08:00 and 21:30 and take your turn working days, evenings, weekends and bank holidays. We need you to be flexible to meet the needs of our customers, but we aim to give you the time you need to spend time doing the things that matter to you!
About you
Youre passionate and motivated. To you providing care and support is more than just a job, you make every customer feel valued and respected. Youre compassionate and use your unique empathy and creativity skills to help our customers reach their full potential. Treating your customers with integrity, you ensure that they maintain their dignity at all times. 
 
Youll help change our customers lives (and our services) for the better by undertaking risk assessments, support planning, goal setting and regular reviews. Youll create bespoke and personalised support plans with and for your customers.  Each plan will look to meet our customers individual potential and long term goals, making sure they meet safeguarding and confidentiality requirements. Youll also have responsibility for coordinating our support workers, apprentices and volunteers, and working collaboratively with external agencies, to assess and engage support for our customers.
 
If youre not the biggest fan of technology, thats not a problem to us. You do need to know that we are quite a high-tech company and most of our application process and mandatory learning are online. However, were glad to support you, if you need help.  Youll be issued with the latest kit, which could include a Surface Pro and mobile and Alertcom (lone working support device) to help you when youre out and about.
 
To make sure our customers are well supported, its important that you have worked in a CQC registered service before. You have a great understanding of professional boundaries, responsibility for safeguarding, risk assessment, confidentiality, and have experience in case management.  We would like to have at least two years experience coordinating a team or at the very least working in the care and support environment. Experience of mental health is essential. We would love it if you have your care certificate, dont worry if you havent, we will ensure you complete this before we allow you to lone work.
 
Having your NVQ 3 would be an advantage too. Dont worry, we wont drop you in the deep end, well help to you understand how things work at Home Group! We offer extensive training both e learning and face-to-face to equip you to carry out your role safely and effectively. You will be competency assessed throughout your induction.

When you join, youll need to have a valid Enhanced Check under the Disclosure and Barring Service. The great news is that we will pay for that to be done if you dont have a transferrable one.
