Physical or Mental Health Needs Support Worker

Permanent, Part Time 22.5 hpw

Penzance, West Cornwall

We have a fabulous opportunity for you to join our awesome team. This is a shiny new service that we have recently been commissioned to deliver. You will work as part of the team to shape, change and influence how the service is going to be provided within our local communities.

 

Our customers are all unique and we support their diverse and specific needs. They are customers with physical or mental health needs or disabilities. Our brilliant customers may have mobility issues, HIV, visual or hearing impairments and/or other long term conditions, such as dementia, learning disabilities, and neurological development disorders, such as autistic disorder, Aspergers syndrome and pervasive development disorder. Alternatively our customers may also have other support needs related to mental health, emotional wellbeing, drugs and alcohol, acquired brain injury, high risk behaviours, contact with the criminal justice system, social isolation, poverty and/or abuse. 

 

We support our customers to connect with other support services and health and social care agencies, where they need this. We deliver support through our Community Outreach Support (which may include holding group support sessions), County Wide Drop-ins, and an advice line. We offer this support in a variety of settings including our customers own accommodation, public spaces and in community based drops- in. Our key aim is to enhance the quality of life for people with health and wellbeing needs; reduce or delay the need for formal care and support; and reduce unplanned use of emergency services.

 

About our team

Youll be working alongside our fabulous team, who are super passionate about supporting customers to reach their goals and celebrating their achievements. The team will be made up of two Senior Client Service Managers, a Referral Coordinator, Health Coaches, Support Coordinators and Support Workers. To us were not just a team were a €œwork family€.

The two Managers are Vicky, who will be managing West Cornwall and has been with Home Group for 15 years & Amanda who will be managing North & East Cornwall and has been with Home Group for 11 years. Between us we have a wealth of knowledge and experience! There will be 14 of us in the dream team and we dont just love to support our customers, were there for each other too! We both have a positive, fun and energised approach to service delivery and team work! We LOVE CAKE!! However we do love exercise too - CrossFit and paddle boarding just to name a couple. We understand the importance of having a good work-life balance and really care about our team.

 

Working hours

The main working hours of the service is going to be Monday Friday 9am to 5pm. However there is an expectation that should you start supporting a customer who needs some help at a weekend or after 5pm, you will be flexible to meet the needs of that individual. We need you to be flexible to meet the needs of our customers, but we aim to give you the time you need to spend time doing the things that matter to you!

 

There is the opportunity for you to earn some extra money by becoming one of our €œEmergency Responders€ who work alongside the On Call managers out of normal hours. However this is on a voluntary basis. Please ask more about this if you are interested.

Looking for a more flexible working pattern? You can also join us on a flexible basis by being part of our bank.  You can work hours that fit around your current commitments or even pick up some additional shifts to your main role to earn some extra cash and get paid on a weekly basis.

 

About you

Youre passionate and motivated to help vulnerable people make positive and lasting changes to their lives. Youll impress us with your ability to work on your own initiative, positively influencing people from all different backgrounds.  You will have experience in safeguarding, risk assessments, confidentiality, support plans, alongside working collaboratively. We help change our customers lives for the better.

 

Youll play your part by assisting in the delivery of tasks that are based on our customers personal support plans, and their needs and aspirations. Youll work with our Support Coordinators and external agencies to assess and engage support for our customers. We empower our customers to develop the independent living skills they need, to successfully move on. Together well make a difference! To make sure our customers are well supported, its important that you have worked in a supported housing environment before.

 

Dont worry, we wont drop you in the deep end, well help you to understand how things work at Home Group! As this is a community based role, you will need to be able to drive, as well as having access to a car insured for business purposes. Dont worry, well pay you expenses from your base of work to cover this use!

 

When you join, youll need to have a valid Enhanced Check under the Disclosure and Barring Service. The great news is that we will pay for that to be done if you dont have a transferrable one.
