Supported Living Coordinator CQC services
Redcar
Permanent, Full Time (37.5 hours per week)
Salary £17,600 to £19,100 pa rising to £23,500 pa through length of service
£9.00 per hour if youd prefer to work on our bank on a casual basis

Were currently putting the finishing touches to our brand spanking new, accommodation based registered CQC service in Redcar.  Our service will support adult customers who have a range of complex needs from Learning Disabilities, Physical and Mental Health issues, supporting them to achieve their long term goals and live their best lives. How brilliant is that!  Youll be based from our brand new building on Grosmont Close, very close to the lovely sea side town of Saltburn by the Sea (you can almost smell the fish and chips).  Plus were in easy reach of local amenities and Redcar on public transport or by car. 
 
Your role
Youll be supporting our customers to identify their aspirations and goals, working closely with them to create support plans which help them to achieve this.  Youll also support your colleagues to deliver on the plans that youve created enabling our customers to live their day to day lives as independently as possible. 
 
Theres lots for you to do, from leading on caseloads, risk assessments, support planning, goal setting as well as regular reviews. Youll coordinate our Supported Living Workers and work collaboratively with external agencies, to assess and engage support for our customers.  Youll find that no two days are the same because everything we do is €œperson centred€ around our customers.  So you might be helping with care (issuing medication or bathing) in the morning, supporting to cook their lunch and a bit of shopping in the afternoon. We even help customers to go on holiday!  Some of our customers may have more complex needs which may include more support with personal care. 
 
Were recruiting a brilliant new team, so youre going to be part of a fabulous new Home Group family.  Youll be reporting to Tara our Registered Manager, who has worked for Home Group for 8 years and has a wealth of experience managing a range of different services.  Outside of work youll find her active in the great outdoors, hiking or climbing the odd mountain here and there or jetting off to sunny climes and working on her tan (lets face it she isnt going to get one here!).    
 
Working hours
Well be providing 24-hour support to our customers, so youll work on a rota basis, normally 7.30am to 10.00pm, taking your turn to work days, evenings, weekends and bank holidays.  You may from time to time be required to do some occasional sleep in shifts during the initial set up of the services or in emergencies. We need you to be flexible to meet the needs of our customers, but in return well give you the time you need to spend time doing the things that matter to you!

About you
Youll have a positive can do attitude and buzzing to help us make a big impact on our customers lives and shape this brilliant brand new service. Your empathy will really shine through as will your ability to empower customers to reach their full potential.  To make sure our customers are well supported, its important that you have worked in a supported housing environment before. Youll have a great understanding of professional boundaries, safeguarding, risk assessment, case management and creating bespoke/personalised support plans, making sure that they meet safeguarding and confidentiality requirements.  Dont worry, we wont drop you in the deep end, well nurture your talent, help you understand how things work at Home Group and youll have a fantastic induction with ongoing learning and support!
 
You will hold a Level 2 certificate or equivalent in Health and Social Care, if you also hold Level 3 this would be the icing on the cake, but dont worry if you dont, as well pay and support you to achieve this.  To make sure that we keep our records straight, and customers safe, it's really important that you are technologically savvy (or at the very least not a technophobe!).  Youll also need to be able to drive and have access to a car.
 
If you are successful, we need you to have an Enhanced Disclosure Check through the Disclosure and Barring Service which well pay for.
