Supported Living Coordinator TorquayPermanent, Full Time (37.5hpw)

We have a fantastic opportunity for a brilliant Supported Living Coordinator like you to join our fab and supportive team. Our CQC registered service provides support to our brilliant customers who have a range of mental health conditions. 


Our staff team support our customers within their homes, helping them gain independence and the skills needed to move forward in the right direction.


Our customers live in properties in both Exeter and Torquay, This role is for our Torquay services but you can always choose to help out in Exeter too.


Youll help our customers identify their aspirations, and work with them to create plans that help them achieve these. You will also help your colleagues to deliver on the plans youve created to achieve excellence. Our aim is to support our customers to live their day to day lives as independently as possible.


Our team will tell you that no two days are ever the same because everything we do is €œperson-centred€ around our customers. You might help with issuing medication in the morning and supporting them to attend college in the afternoon. We even help customers to go on holiday! Like our team, were sure youll find our role ever so rewarding.


About our teamOur team describe themselves as really friendly and approachable. In fact when new members have joined us in the past, its been said that they felt welcomed by colleagues and our customers from day one!


Were also super passionate about supporting customers. With colleagues as warm and team spirited as ours, were not just a team were a work family! Our service manager is Adam, and Becky oversees our Torquay and Exeter services. We have 3 properties, which make up our DECRS or Devon Enhanced Community Recovery Service. There are around 15 of us now in the team and we dont just love to support our customers, were there for each other too!


Working hoursWe provide 24-hour support to our customers each day and every day on a rota basis between the hours of 07:00 and 22:00. The shift times vary day to day so don't panic you won't just be doing long days! Youll take your turn working days, evenings, weekends and bank holidays. We need you to be flexible to meet the needs of our customers, but we aim to give you the time you need to spend time doing the things that matter to you!


BankLooking for a more flexible working pattern? You can also join us on a flexible basis by being part of our bank. You can work hours that fit around your current commitments or even pick up some additional shifts to your main role to earn some extra cash and get paid on a weekly basis.


About youYoure passionate and motivated. To you providing care and support is more than just a job, you make every customer feel valued and respected. Youre compassionate, and use your unique empathy and creativity skills to help our customers reach their full potential. Treating your customers with integrity, you ensure that they maintain their dignity at all times.


Youll help change our customers lives (and our services) for the better by undertaking risk assessments, support planning, goal setting and regular reviews. Youll create bespoke and personalised support plans with and for your customers.  Each plan will look to meet our customers individual potential and long term goals, making sure they meet safeguarding and confidentiality requirements. Youll also have responsibility for coordinating our support workers, apprentices and volunteers, and working collaboratively with external agencies, to assess and engage support for our customers.


To make sure that we keep our records straight, and customers safe, it's really important that you are technologically savvy (or at the very least not a technophobe!) Youll be using the latest kit shared between the staff in the service office and as most of our systems are computer based we will be more than happy to support you.


To make sure our customers are well supported, its important that you have worked in a mental health recovery environment before. You have a great understanding of professional boundaries, responsibility for safeguarding, risk assessment, confidentiality and have experience in case management.  Dont worry, we wont drop you in the deep end, well help to you understand how things work at Home Group!


As this is a community based role, youll need to travel around to spend time with your customers. You could do this role using public transport as we are located close to the centre of Exeter. If you drive and have access to a car that would also be amazing! (We pay your expenses to cover this use if you're travelling during your working hours!)


If you are successful, we need you to have an Enhanced Disclosure Check through the Disclosure and Barring Service which well pay for.
