Job Description

We are currently recruiting for a Care Practitioner in the Sale location!

Hays Social Care are looking for confident and caring adult support workers / care practitioners who want to help make a positive difference to vulnerable individuals' lives.

We are looking for an individual who can provide person-centred support for individuals who have a range of physical and learning disabilities, including learning disabilities and complex care needs. You will need to combine your professional experience and skills to work alongside service users and the core staff team in order to improve their quality of life and promote their independence.

You will need to be confident in working with individuals who have a range of complex needs and have the genuine desire to support the work of the permanent members of staff. If you are working at the service, you will be expected to develop a strong and lasting relationship with the service users. It is vital to encourage the service users and prompt them when necessary. You will be required to complete daily records, including service user portfolios, logbooks, and behaviour monitoring forms. The care practitioner's role involves pushing wheelchairs and using hoists. Therefore, the post holder must be physically capable to complete these tasks.
The post holder will be provided with ongoing professional training and receive guidance and support from experienced managers.

Key duties within this role will include:

  • Complete health care tasks to a high standard
  • Be able to communicate effectively with people
  • Provide support for the team and for staff who are in their probation period
  • Providing personal care, supporting individuals with all identified self-help skills, helping people who are showing signs of distressed behaviour, following support plans, showing empathy and contributing to multidisciplinary meetings
  • Assume shift leader responsibilities when directed and fulfil these responsibilities fully in line with the competency assessment framework.
  • Carrying out all health care procedures in line with the people's support plan.
  • Ensuring the health and safety of service users, staff and other people in the service is a priority.
  • Maintain accurate and up-to-date records of the work undertaken by service users using both written and electronic media.
  • Demonstrate effective time management and be able to work without direct supervision, including lone working as appropriate.
  • Ensure you are knowledgeable about Stockdale's policies and procedures and undertake any task deemed appropriate to the post and the remit of the service.
  • Comply with the standards of conduct and practice established by regulatory bodies, including CQC and OFSTED, and ensure that you adhere to them at all times.

  • To be considered for Support Worker roles you must:

  • Have at least 6 months of social care experience within the last 2 years
  • Be able to provide two references from your most recent employers.
  • Have experience working with children in a residential setting or supported living with young people with learning difficulties, ASD, challenging behaviour, and mental health.
