Your new company
We are currently working with Swansea Council to recruit a Qualified Social Worker to join their Home First Team. As part of the Home First Team, you will be focussing on promoting independence and supporting individuals with enhanced needs who are leaving the hospital to return home. You will be responsible for planning, implementing, reviewing, and evaluating the outcomes for individuals, whilst the focus is on understanding the unique needs of each person and involving them in decision-making.
Your new role
This role is a 3-month, full-time contract, working 37 hours per week, split between homework and office. The rate will be £35 per hour (umbrella).
What you'll need to succeed
- Social Work degree and registered with Social Care Wales
- Full UK driving licence
- At least 3 years experience
