Support Coordinator
Hartlepool, Cleveland
Permanent, full time (37.5 hpw)
Salary of £19,000 to £20,125 pa, pro rata
From £9.26 per hour if youd prefer to work on our bank on a casual basis

We have a fabulous opportunity for you to join our awesome team. We support our brilliant customers who have multiple, complex needs. They live in Scott Grange, which is our beautiful converted building, as well as in flats across the wider Hartlepool area. We help our customers with many aspects of their lives in a unique way, enabling them to tackle difficulties head-on and to thrive in the future.
 
About our team
Youll be working alongside our fabulous team who are super passionate about supporting customers to reach their goals and celebrating their achievements. To us were not just a team were a €œwork family€. Our manager is Dom, who has been with Home Group for 3 years. He spent 10 years in housing and loves almost nothing more than solving problems with the awesome bunch of people at Scotty. The team includes an amazing mix of Love Island expert, Trekkies, Mrs Hinch lovers, who all have in common the passion to support our customers in their endeavours.
 
Working hours
We provide 24-hour support to our customers each day and every day. Youll work on a rota basis mostly between 8am and 10pm and take your turn working days, evenings, weekends and bank holidays. We need you to be flexible to meet the needs of our customers, but we aim to give you the time you need to spend it on doing the things that matter to you!

Looking for a more flexible working pattern? You can also join us on a flexible basis by being part of our bank.  You can work hours that fit around your current commitments or even pick up some additional shifts to your main role to earn some extra cash and get paid on a weekly basis.
 
About you
Youre passionate and motivated to help vulnerable people make positive and lasting changes to their lives. Youll impress us with your ability to work on your own initiative, positively influencing people from all different backgrounds. Youll help change our customers lives (and our services) for the better by undertaking risk assessments, support planning, support delivery, housing management, goal setting and regular reviews. Youll create bespoke and personalised support plans with and for your customers.  Each plan will look to meet our customers individual needs and aspirations, making sure they meet safeguarding and confidentiality requirements. Youll also have responsibility for coordinating workload, and working collaboratively with external agencies, to assess and engage support for our customers.
 
We empower our customers to develop the independent living skills they need, to successfully move on. Together well make a difference!To make sure that we keep our records straight, and customers safe, it's really important that you are technologically savvy (or at the very least not a technophobe!) Youll be given the latest IT kit, which could include a Surface Pro tablet and smart phone, supported by our new online integrated IT system which means that you dont have to be in the office as you can complete support plans on the go.

As this is a community based role, you need to be able to drive, as well as having access to a car insured for business purposes. Dont worry, well pay you expenses to cover this use!
 
When you join, youll need to have a valid Enhanced Check under the Disclosure and Barring Service. The great news is that we will pay for that to be done if you dont have a transferrable one.
