Support Coordinator

Cleveland, Saltburn

Full Time (37.5 hours per week), 12 Months Fixed term

Salary £19,300 to £20,250 pa rising to £24,300 through length of service, plus brilliant benefits!

We have a fabulous opportunity for you to join our awesome team. We support our brilliant customers who have mental health and/or learning disability support needs.

They live in the Redcar & Cleveland area in the local community in dispersed properties which can range from 1-2-bedroom flats as well as houses.

We help our customers enjoy the best quality of life they can, support to maintain social inclusion, access and understand benefits, budgeting, access to voluntary work, accessing local services or day centres and supporting to liaise with relevant stake holders.

Working hours

You’ll work on a rota basis between the hours of 09:00 – 20:00 and take your turn working days, evenings, weekends and bank holidays. We need you to be flexible to meet the needs of our customers, but we aim to give you the time you need to spend time doing the things that matter to you!

About you

You’re passionate and motivated to help vulnerable people make positive and lasting changes to their lives. You’ll impress us with your ability to work on your own initiative, positively influencing people from all different backgrounds. 

You’ll help change our customer’s lives (and our services) for the better by undertaking risk assessments, support planning, goal setting and regular reviews. You’ll create bespoke and personalised support plans with and for your customers.  Each plan will look to meet our customer’s individual needs and aspirations, making sure they meet safeguarding and confidentiality requirements. You’ll also have responsibility for coordinating our support workers, apprentices and volunteers, and working collaboratively with external agencies, to assess and engage support for our customers.

About our team

You’ll be working alongside our fabulous team who are super passionate about supporting customers to reach their goals and celebrating their achievements. To us were not just a team we’re a ‘work family’. Our manager is Mark who has worked in health and social care for 9 years. With a background in CQC regulated services and a wealth of experience with autism, learning disability and mental health support needs. Mark works alongside the support team on the front line to ensure support is always available. There are 7 of us in the team and we don’t just love to support our customers, we’re there for each other too! This will be a brand-new team consisting of a Senior Client Service Manager, Client Service Manager and 5 x Support Co-ordinators!

If you’re not the biggest fan of technology, that’s not a problem to us. You do need to know that we are quite a high tech company and most of our application process and mandatory learning are online. However we’re glad to support you, if you need help.  

To make sure our customers are well supported, it’s important that you have worked in a mental health or learning disability supported living environment before. Don’t worry, we won’t drop you in the deep end, we’ll help to you understand how things work at Home Group!

As this is a community based role, you need to be able to drive, as well as having access to a car insured for business purposes. Don’t worry, we’ll pay you expenses to cover this use!

When you join, you’ll need to have a valid Enhanced Check under the Disclosure and Barring Service. The great news is that we will pay for that to be done if you don’t have a transferrable one.
