Brilliant work/life balance, plus, brilliant benefits!

Brilliant outcomes through integrated health, housing and care. That’s when it hits home

This is your chance to be part of something brilliant at Home Group, where clinical interventions work hand in hand with daily support. You’ll be joining us on our exciting journey to become market leaders in New Models of Care by the end of 2021. This means we’re not providing your traditional supported housing services but rather trailblazing the way in delivering brilliant long-term outcomes for vulnerable people through integrated health, housing and care.

We have a brilliant opportunity for an Occupational Therapist to join our supported housing service at Buller Court, which provides support to adults with enduring mental health and/or autism. You’ll be supporting our customers to identify the things they want and aspire to do in their lives. This will include the assessment and management of a range of complex occupational therapy needs, diverse presentations and a range of mental health conditions. You’ll identify and facilitate collaboratively skills development programmes and support customers to achieve their goals through engaging in meaningful and purposeful everyday activities important to them and for them. You’ll work closely with our Support Workers to deliver personalised interventions, formulation meetings and from this develop goal-based recovery and wellbeing focused plans, as well as regularly reviewing customer progress.

You will lead practice development, providing ongoing clinical supervision, reflective practice and coaching to our Managers and Support Workers to ensure that high quality individualised care is delivered which ensures safety, recovery, wellbeing and maximises customers’ ability to live independently and feel enabled. You will also be a key part of the team following incidents to ensure everyone really understands what happened, confidently challenging practice and how identifying how we can learn from it.

You will inspire others and advocate a philosophy of care that is person centred, maintains dignity, independence and ensures it is right for that person at that time. You will support the introduction and embedding of our new ‘LIFE’ support model, including FACE risk assessments and PIE.

Your reach will extend beyond the team to develop standards and quality outcomes and evidence improvements and clinical outcomes for Home Group more widely and for our Commissioners to inform future service delivery. This is a role with the potential for significant influence!

About our team

On a day to day basis, you’ll report to Penny, our Operations Manager who has worked in our supported services for 17 years! Our Clinical Practice team will provide support and clinical supervision to you while you’ll regularly work closely with the Service Manager Jay. Jay’s been here at Home Group for nearly a decade and has worked her way up from frontline support to her Senior Client Service Manager role! When not in work Jay is usually listening to heavy metal, or looking after her cat Smudge, but in work she’ll support you to settle into your new role. To us we’re not just a team we’re a ‘work family’. We don’t just love to support our customers, we’re there for each other too!

Working flexibly

It’s our unwritten rule that you attend the important appointments in life, be it your child’s school play, your partner’s graduation, or to get your new fridge delivered. You can elect to fit these around your working day or use your flexi to take the time off. As our service is 24/7 you really can fit your hours in to suit both yourself and the service. Our focus is on getting the job done, not your working hours.
