Support Coordinator

(Referral Coordinator, North Tyneside Partnership)

Permanent Full Time 37.5 hpw

We are looking for an energised and passionate Referrals Coordinator to join our awesome team. We support our brilliant customers through the North Tyneside Partnership. We provide housing, health and wellbeing services to customers within North Tyneside, with mental health and complex needs. As lead contract holder, youll work closely with our partners, Karbon Homes, Turning Point and Tyne Housing, delivering outstanding support.

 

Collectively we provide 100 units of accommodation, 50 units of floating support, as well as our Hospital Discharge service, aimed at supporting customers with chronic alcohol misuse and with multiple hospital admissions.

 

As Referrals Coordinator youll play a crucial role in the partnership and be the main point of contact for all referrals, working closely with the North Tyneside Gateway team, our partners and the team at Railway Terrace.  Carrying out initial assessments, risk assessments, customer induction, and setting up HB claims are key to the role, and require someone who has good knowledge and experience of relevant support procedures.  Alongside this, someone, who has the ability to develop strong working relationships with key stakeholders is paramount.

This post is for a Referral Coordinator level 3 role, and if successful youll be part of our team at Railway Terrace, and be managed by one of the CSMs there. 

Youll be working primarily Monday to Friday 9-5. Well also need you to be flexible to meet the needs of our customers/stakeholders outside of these hours, and to support the wider Railway Terrace team, but we aim to give you the time you need to spend it doing the things that matter to you!

 

You will be given the latest IT kit, which could include a Surface Pro tablet and smart phone, supported by our new online integrated IT system, which means that you dont have to be in the office as you can complete support plans on the go. Youll also be paid between customers visit and for your mileage too.  

 

A bit about you

To flourish in the role, you will be a like-minded dedicated person, who can demonstrate compassion, empathy and creativity in your approach and use your unique skills in enabling customers to reach their full potential. 

With previous experience of working with vulnerable people in a support or care environment, you will have a great understanding of professional boundaries, safeguarding, risk assessment, confidentiality and be experienced in case management.  Its also important that you can work in holistic way, building partnerships with external agencies and that you have strong IT skills too.

 

When you join, youll need to have a valid Enhanced Check under the Disclosure and Barring Service. The great news is that we will pay for that to be done if you dont have a transferrable one.
